Do you need a Birth
Certificate or Death Record? You may request
certificates by phone by calling 1-877-817-7362
(option 3). To use the call in feature you must
have a major credit card. There will be a credit
card charge fee in addition to the cost of the
certificate requested. Average turn around time
for a phone order is four (4) or (5) days plus
mail time.
You can download a printable application by
visiting the state website at:
http://chfs.ky.gov/dph/vital/
Mail in request average thirty (30) business days.
These applications may also be picked up at our
office.
You may walk in at Frankfort and be processed in
an average of one (1) hour. Check or money order
is required at the time of your visit.
